Continuum of Care
Program Description
The Continuum of Care is a community-based, long-range plan that addresses the needs of homeless persons in order to help them reach maximum self-sufficiency. The continuum includes programs that provide emergency services, transitional services, and permanent supportive housing to people who qualify for the various programs designed to help people who need assistance to obtain and sustain a decent housing environment.
Funding Source
The Continuum of Care is funded by the U.S. Department of Housing and Urban Development (HUD) and is viewed as a comprehensive and strategic approach to addressing homelessness.
Eligibility Requirements
Projects that address identified needs are selected by the South Dakota Housing for the Homeless Consortium (SDHHC) membership for inclusion in the statewide application. The SDHHC, which is a statewide organization of nonprofit and private businesses, disability service organizations, local community representatives, public housing authorities, private landlords, former homeless persons, housing developers, regional community action agencies, and state agencies, submits a single statewide application to access this federal resource annually.
Eligible Activities
Continuum of Care Homeless Assistance Programs include:
- Supportive Housing Program
Provides housing, including housing units and group quarters, that has a supportive environment and includes a planned service component. - Shelter Plus Care Program
Provides grants for rental assistance for homeless persons with disabilities through four components: tenant, sponsor, project, and single room occupancy rental assistance. - Single Room Occupancy Program
Provides rental assistance on behalf of homeless individuals in connection with moderate rehabilitation of SRO dwellings.

Application Procedure
A statewide application is submitted by SDHHC annually to HUD. This application outlines South Dakota's strategic approach to addressing homelessness.
